THEORIES AND MECHANISMS OF HEALTH INSURANCE: WORLD EXPERIENCE AND IMPLEMENTATION IN UKRAINE
Abstract
The paper examines the theoretical and practical dimensions of health insurance system development and operation, emphasising the international experience of post-war healthcare reforms and its significance for Ukraine. The study organises the essential characteristics of different healthcare financing models, such as the Bismarck, Beveridge, and national insurance models, and offers a comparative analysis of their institutional structures, funding sources, population coverage levels, and public-private interaction mechanisms. Particular emphasis is placed on the experiences of nations including the United Kingdom, Germany, South Korea, Croatia, and Bosnia and Herzegovina, which reformed their healthcare systems in the post-conflict era to guarantee equitable access, long-term sustainability, transparency, and social cohesion. These nations exemplify the significance of integrating governmental regulation, employer involvement, and individual contributions to attain stability and responsiveness in healthcare financing. The essay delineates fundamental issues confronting the Ukrainian healthcare system amid military conflict, encompassing infrastructure degradation, workforce emigration, constrained fiscal capacity, and eroding public trust in institutions. The authors present essential proposals for the phased introduction of a mixed health insurance model in Ukraine, grounded in international research. This encompasses the establishment of a transparent and accountable national insurance fund, gradual expansion of compulsory coverage, enhancement of the primary healthcare network, incentivisation of employer-based contributions via fiscal measures, and advancement of the digital transformation of insurance administration. The participation of international partners and donor organisations is highlighted as a crucial element for facilitating institutional reforms and securing financial stability during the initial stages of post-war rebuilding. The proposed reforms are anticipated to enhance the modernisation, equity, and resilience of Ukraine’s healthcare system, establishing a foundation for sustainable health policy in the long run.
